Kindergarten Vision Screening Record.

Your incoming kindergarten student MUST have documentation that his/her vision and eyes have been examined by either a licensed eye care practitioner or a medical provider.

A vision screen should be part of the kindergarten physical examination. Provide
either the completed Health Appraisal form (with vision screen results)
OR a signed statement from the medical provider with date of vision screen and results.
